Clinician Performance T-Tool ™
The purpose of the Clinician Performance Trending Tools (T-Tool ™) is to assist home health agencies with collecting, tracking and trending individual clinician performance measure data. This T-Tool includes a quality of care (QoC) Star Measures tab and an HHCAHPS Star Measures tab for monthly and quarterly performance data collection. It provides color-coded at-a-glance tracking and trending capability based on quarterly CMS State and Agency Averages, and monthly clinician performance data. Using this T-Tool is a proactive approach to transforming data into useful information that can assist with identifying individual clinician performance gaps and the need to develop and implement performance improvement activities.
