Star Measures Monthly Quality and HHCAHPS Dashboard Trending Tool ™
The purpose of this Star Measure Quality and HHCAHPS Dashboard Monthly Trending Tool (T-Tool ™) is to assist home health agencies with collecting, tracking and trending quarterly CMS performance measure data. This T-Tool includes a quality of care (QoC) Star Measures tab and an HHCAHPS Star Measures tab for monthly and quarterly performance data collection. It provides color-coded at-a-glance tracking and trending capability based on quarterly CMS State and CMS National Averages, and monthly performance data. Using this T-Tool is a proactive approach to transforming data into useful information that can assist with identifying performance gaps and developing and implementing performance improvement activities.
