Timely data analysis is essential to improving quality outcomes. The purpose of this Monthly HHVBP Large Cohort - Data Collection T-Tool ™ is to assist home health agencies with collecting, tracking and trending HHVBP IPR performance measure data. This T-Tool includes (12) tabs for monthly data collection, and a trended data tab that provides color-coded at-a-glance tracking and trending capability. This T-Tool will provide an estimated calculation of your Achievement Points, Improvement Points, Care Points, and Weighted Points for each performance measure, with color-coded at-a-glance comparison to the CMS measure weights. Using this T-Tool is a proactive approach to transforming data into useful information that can assist with identifying performance gaps and developing and implementing performance improvement activities.
